Pupil Referral Form

A referral to our service must come through the school.

To refer a pupil to our service for individual support, please complete the confidential referral form and attach any relevant documents such as examples of work or recent reports. You will need to obtain a purchase order number from your school’s finance team before completing this form.

All visits are subject to receiving written parental consent prior to the date of the visit.

What will you need?

  • Pupil details including the individual UPN
  • Know which type of service you require — if you are unsure please speak to one of the team to identify your needs ahead of completing the form.
  • Purchase Order Number from your school finance team
  • Information about the child’s history, current interventions and attainment. Please also upload any relevant documents.
  • Parental Consent Form signed by the child’s parent/carer. This does not need to be attached to the referral form; however, it must be received by the team prior to the visit.

What to expect

  1. Obtain parental consent, collate any relevant work samples/reports and pupil’s details (including purchase order number) for each pupil you are referring.
  2. Complete the pupil referral form(s) and upload any relevant documents.
  3. A DOS Specialist Teacher will be in touch to book a date to visit the pupil(s).
  4. Following the visit, the Specialist Teacher will produce a support plan/assessment report and return this to the SENCo/contact in a timely manner. Please note: wait times for paperwork can vary depending on workload; however, we aim to return plans/reports to the school within 3–4 weeks.
  5. School to implement/try recommendations/resources included in the plan/report.
Written parental consent is required for DOS involvement. Please provide parent details in order for us to issue the consent form.
